Notice Regarding Delivery of Electronic Records

As used in this notice “I” and “me” shall be understood to refer to a member, officer, director or employee.  By signing a Consent to Receive Electronic Records, I consent to MANA’s use of an electronic record to provide and make available to me information that is required to be provided or made available to me in writing pursuant to the California Corporations Code or any other applicable law.  I further acknowledge that my consent shall remain in effect until I withdraw the same by delivering to MANA notice in writing (the “Notice of Withdrawal”) to that effect. The Notice of Withdrawal shall be delivered to MANA’s address as posted on MANA’s website by an overnight courier that will issue proof of delivery or by certified mail return receipt requested. I shall keep MANA informed of my current e-mail address. Should I change my e-mail address, I will inform MANA in writing at: manasd@manasd.org.

I further acknowledge that I was informed of the following:

a) My right, after consent and upon request, to have the record be provided or made available on paper or in non-electronic form;

b) My right to withdraw this consent and elect to have the record be provided or made available on paper or in non-electronic form;

c) This consent applies to any and all communications by and between me and MANA; and

d) For access to and retention of electronic records, you will need a computer with internet access.

MANA reserves the right to modify this Policy at any time.

Consent to Receive Electronic Records from MANA de San Diego

I hereby acknowledge that I have read the “Notice Regarding Delivery of Electronic Records” and hereby consent to receive electronic records and communications as set forth in the Notice to the following e-mail address: manasd@manasd.org.  I further agree that in the event that the e-mail address provided in this consent is no longer current, I shall follow the guidelines set forth in the Notice to provide MANA de San Diego with my current e-mail address.
